Saw this bayonet on eBay. J Hawk, can you or anyone else translate it? There are a lot of Kanji for just a "turn in return tag"

I Think, that is real tag and kanji.
Name and old address are writing.
Address: Shimane prefecture-------------.
Name: Shiga/Itoga Hikotaro.
